Appointment Form

Sessions are usually for 50-60 minutes. A 24-hour cancellation policy is required. That is, the client is required to cancel the appointment at least 24 hours prior to their scheduled appointment. Paperwork will be provided at the initial appointment. Payment is made at the time of service itself.
